Illustrative, not evidence.

Scenario. In a research reactor, a loss of power releases the control rods so they drop into the core. This is the defined safe state. A test cuts the power to show that the rods reach it. Loading fuel into the core is critical: a fuel element in the wrong position can cause harm that cannot be undone. So the procedure needs two authorized operators at the moment of action, and the system refuses the move unless a second operator approves.

  • P37: The design defines a safe state and makes failure move there. Failure injection, a power cut, shows that the rods reach the safe state.
  • P42: Fuel loading is a critical action whose harm cannot be undone. The system enforces a second approval from a second authorized person.
